This form can be obtained from the Membership Secretary, or the British Sub Aqua Club Website, or the UK Sports Diving Medical Council Website. You can also download the more detailed form for completion by a diving medical referee from these sites, or ask the Membership Secretary for a copy.

If you answer yes to any of the questions on the self declaration medical form you will be asked to contact a diving medical referee who will be able to give you more information about how any medical conditions may affect your diving. You may need a medical with a diving doctor, or they may be able to endorse your form so you are cleared to dive just by asking you for more information.
